Resumen: En la evaluación de la carcinomatosis peritoneal (CP) la imagenología contribuye al diagnóstico preciso de compromiso peritoneal, aunque su eficacia es aún limitada. La tomografía computada (TC) es el estudio de imagen más utilizado y el PET-TC es cada vez más solicitado en la valoración de diferentes neoplasias. El objetivo del presente trabajo es evaluar el rendimiento de la TC y el PET-TC en el diagnóstico de CP en tumores digestivos y ginecológicos. Material y método: estudio retrospectivo, observacional, de 11 casos operados de CP en los que previamente se había realizado TC o PET-TC, o ambos. Se analizan los resultados de los estudios imagenológicos por separado y en conjunto, y se comparan con los hallazgos intraoperatorios y anatomopatológicos. Resultados: la TC fue concordante en 8 de 10 casos (S 78%, E 100%, VPP 100%, VPN 33%), mientras que el PET-TC lo fue en 9 de 11 pacientes (S 100%, VPP 81%). Al considerar en conjunto la TC y el PET-TC, la sensibilidad fue de 100% y el VPP de 82%. Conclusiones: en los resultados obtenidos se constata una subestimación de la CP en la imagenología a causa de las características de los nódulos y su patrón de captación. En el PET-TC, un SUV > 6 podría aumentar la probabilidad diagnóstica de carcinomatosis. Esta breve serie de casos permite sugerir que TC y el 18FGD PET-TC deben ser solicitados y analizados en forma conjunta para incrementar su rendimiento, aunque la exploración quirúrgica continúa siendo el método más eficaz para el diagnóstico y evaluación de la CP.


Summary: Imagenology contributes to the accurate diagnosis of peritoneal compromise in the assessment of peritoneal carcinomatosis, although its effectiveness is still limited. CT is the most frequently used study and PET/CT is graduallly more requested in the assessment of different neoplasms. This study aims to assess performance of CT and PET/CT for diagnosis of peritoneal carcinomatosis in digestive and gynecological tumours. Method: retrospective, observational study of 11 cases operated on peritoneal carcinomatosis after a CT and/or PET/CT had been performed. The results of both image studies were analysed separately and altogether and they were subsequently compared to intraoperative and pathology studies. Results: CT matched findings in 8 out of 10 cases (sensitivity 78%, specificity 100%, positive predictive value 100%, negative predictive value 33%), whereas PET/TC matched in 9 out of 11 patients (sensitivity 100%, PPV 81%). When considered together, sensitivity was 100% y el PPV de 82%. Conclusions: the results of the study confirm imagenology studies overestimate peritoneal carcinomatosis based on the characteristics of nodules and its uptake pattern. In CT/PET, SUV> 6 could increase the probability of diagnosing carcinomatosis. This small series of cases enables our suggesting CT and 18F-FDG PET-TC to be requested and analysed together to increase its efficiency, although surgical exploration continues to be the most effective method to diagnose and assess peritoneal carcinomatosis.


Resumo: Na avaliação da carcinomatose peritoneal (CP) as técnicas de imagem contribuem ao diagnóstico preciso do compromisso peritoneal, embora sua eficácia ainda seja limitada. A tomografia computadorizada (TC) é o estudo de imagens mais utilizado e a tomografia computadorizada por emissão de pósitrons (PET-TC) é cada vez mais solicitado na avaliação de diferentes neoplasias. O objetivo do presente trabalho, é avaliar o rendimento da TC e do PET-TC no diagnóstico de CP em tumores digestivos e ginecológicos. Material e método: estudo retrospectivo, observacional de 11 casos de carcinomatose peritoneal operados nos quais previamente se havia realizado TC e/ou PET-TC. Os resultados de ambos estudos foram estudados isoladamente e em conjunto e foram comparados com os achados intraoperatórios e histopatológicos. Resultados: a TC foi concordante em 8 de 10 casos (S 78%, E 100%, VPP 100%, VPN 33%), e o PET-TC em 9 de 11 pacientes (S 100%, VPP 81%). A sensibilidade foi de 100% e o VPP de 82% quando ambos estudos foram analisados conjuntamente. Conclusoes: observa-se nos resultados uma subestimação da CP nas técnicas de imagem, devido às características dos nódulos e seu padrão de captação. No PET-TC, um SUV > 6 poderia aumentar a probabilidade de diagnosticar carcinomatose. Esta breve série de casos permite sugerir que a TC e o 18FGD PET-TC devem ser solicitados e analisados em forma conjunta para aumentar o rendimento. No entanto a exploração cirúrgica continua sendo o método mais eficaz para o diagnóstico e avaliação da CP.

ABSTRACT Introduction Cancer is one of the most important leading cause of death in man and woman in the world. The occurrence of new cancer has become more frequent in recent years due to strict screening protocols and occupational and environmental exposure to carcinogens. The incidence of secondary malignancies has also increased due to close medical follow-up and advanced age. Herein, we report a case and its management diagnosed as synchronous peritoneal malignant mesothelioma and muscle-invasive urothelial carcinoma. Case Description A 71-year-old male presented with macroscopic hematuria and abdominal distension increasing gradually. A contrast enhanced computerized tomography demonstrated bladder mass and diffuse ascites with nodular peritoneal thickening and umbilical mass. He was treated with the multidisciplinary team working including urologist, medical oncologist and general surgeon. Conclusions To our knowledge, this is the first case of peritoneal malign mesothelioma with synchronous muscle-invasive urothelial carcinoma. Because of the rarity of this condition, there is still no consensus on the definitive treatment protocols, yet. Individualized treatment with multidisciplinary close follow-up might improve the survival outcomes.

ABSTRACT

SUMMARY Tuberculous peritonitis is one of the most common causes of exudative ascites, especially in the young, and is an important cause of extra-pulmonary disease. However, tuberculous peritonitis is challenging to diagnose because there are no pathognomonic clinical features or imaging findings. Therefore, it is commonly misdiagnosed as another type of peritoneal disease, especially so in elderly patients with malignant disease. In this report, we described two cases of tuberculous peritonitis that were observed after intestinal perforation in elderly patients with malignancies. These diagnoses were established by laparoscopic peritoneal biopsy or AFB cultures of the ascitic fluid. Both patients were treated with anti-TB medications.

Summary Introduction: Pseudomyxoma peritonei (PMP) is a rare clinical condition, with an incidence of 1-2 cases per million, characterized by the dissemination of mucinous implants on the peritoneal surface and progressive gelatinous ascites. Although it usually presents an indolent behavior, its non-specific clinical presentation contributes to many cases remaining undiagnosed until a laparotomy is performed. With late diagnosis, performed after a long period of clinical deterioration and disease progression, it is common to find complications such as the formation of intestinal fistulas and obstruction. Method: Review of the medical record and search for references in the Medline, Lilacs, SciELO and MD Consult databases. Results: There are rare case reports found in the literature demonstrating atypical PMP presentations. Our report is that of a 17-year-old adolescent with a sporadic tumor diagnosed in a primary site in the transverse colon, contrary to data commonly found in the literature that mention a more frequent occurrence in women in the fifth decade of life and with a primary site in the ovary and appendix. The development of mucinous adenocarcinoma is rare in the pediatric population, and topography in the transverse colon and non-familial sporadic pattern are unusual. Conclusion: The case reported not only raises awareness about the atypical presentations of the disease, but also emphasizes the use of imaging examinations for diagnosis, which has an important impact on prognosis and survival if performed timely.


Resumo Introdução: O pseudomixoma peritoneal (PMP) é uma condição clinica rara, com incidência de 1-2 casos por milhão, caracterizada pela disseminação de implantes de natureza mucinosa pela superfície peritoneal e acúmulo progressivo de ascite gelatinosa. Embora apresente geralmente um comportamento indolente, a apresentação clínica inespecífica contribui para que muitos casos permaneçam sem diagnóstico até a realização de laparotomia. Com o diagnóstico tardio, realizado após um longo período de deterioração clínica e progressão de doença, é comum encontrar complicações, como a formação de fístulas e obstruções intestinais. Método: Revisão do prontuário médico e pesquisa bibliográfica nas bases de dados Medline, Lilacs, SciELO e MD Consult. Resultados: São raros os relatos de caso encontrados na literatura que demonstram apresentações atípicas do PMP. O presente estudo apresenta o caso de um adolescente com 17 anos ao diagnóstico e sítio primário no colón transverso com tumor esporádico, contrário aos dados comumente encontrados na literatura, que referem acometimento mais comum em mulheres na quinta década de vida e com sítio primário em ovário e apêndice. O desenvolvimento de adenocarcinoma mucinoso é raro na população pediátrica e a topografia no cólon transverso e padrão esporádico não familial também são pouco usuais. Conclusão: O caso relatado alerta para as apresentações atípicas da doença e enfatiza o uso de exames de imagem para o diagnóstico, que, se realizado precocemente, impacta de maneira importante o prognóstico e a sobrevida.

ABSTRACT

Benign intraperitoneal metastatic leiomyomatosis is a rare benign disease that is observed when a leiomyoma is present in the peritoneal surface. Women who have undergone hysterectomy for leiomyomas are most commonly affected. Patients are usually asymptomatic at presentation, being frequently an incidental finding in imaging studies. Ultrasound and CT play an important role in the diagnosis. The lesions are histologically identical to their uterine counterparts. There are different theories about the pathogenesis of the disease, including peritoneal seeding after laparoscopic hysterectomy. Others support the hypothesis of multiple independent foci of smooth muscle proliferation. Treatment, as in uterine leiomyomatosis, is generally conservative. We report a 53-year-old hysterectomized woman with intraperitoneal leiomyomas detected in a routine physical examination as mobile abdominal masses who underwent successful laparoscopic resection.

ABSTRACT

OBJECTIVE: The aim of our study was to compare the accuracy of CT and 18F-FDG PET for detecting peritoneal metastasis in patients with gastric carcinoma. MATERIALS AND METHODS: One-hundred-twelve patients who underwent a histologic confirmative exam or treatment (laparotomy, n = 107; diagnostic laparoscopy, n = 4; peritoneal washing cytology, n = 1) were retrospectively enrolled. All the patients underwent CT and 18F-FDG PET scanning for their preoperative evaluation. The sensitivities, specificities and accuracies of CT and 18F-FDG PET imaging for the detection of peritoneal metastasis were calculated and then compared using Fisher's exact probability test (p < 0.05), on the basis of the original preoperative reports. In addition, two board-certified radiologists and two board-certified nuclear medicine physicians independently reviewed the CT and PET scans, respectively. A receiver-operating characteristic curve analysis was performed to compare the diagnostic performance of CT and 18F-FDG PET imaging for detecting peritoneal metastasis. RESULTS: Based on the original preoperative reports, CT and 18F-FDG PET showed sensitivities of 76.5% and 35.3% (p = 0.037), specificities of 91.6% and 98.9% (p = 0.035), respectively, and equal accuracies of 89.3% (p = 1.0). The receptor operating characteristics curve analysis showed a significantly higher diagnostic performance for CT (Az = 0.878) than for PET (Az = 0.686) (p = 0.004). The interobserver agreement for detecting peritoneal metastasis was good (κ value = 0.684) for CT and moderate (κ value = 0.460) for PET. CONCLUSION: For the detection of peritoneal metastasis, CT was more sensitive and showed a higher diagnostic performance than PET, although CT had a relatively lower specificity than did PET.

ABSTRACT

The malignant peritoneal mesothelioma is rare and its symptomatology is not very specific, the radiological examinations are of a great interest in the diagnosis of this rare pathology like in the assessment of extension and the search of metastasis. We report a malignant case of peritoneal mesothelioma in a 11 year old child diagnosed with echography and abdomino-pelvic TDM and confirmed histologicaly. The peritoneal localization of the malignant mesothelioma is rare and seldom occurs in the child, its diagnosis must be evoked in front of an abdominal tumor with peritoneal and mesenteric attack in the ascites. The diagnosis of certainty is histological, the prognosis remains pejoratif

ABSTRACT

Leiomyosarcoma is a rare tumor that originates in the smooth muscle, usually in the gastrointestinal tract, the retroperitoneum and the genitourinary tract. The omentum has been rarely reported as a primary site of occurrence with leiomyosarcoma. A 72 year-old woman, with palpable mass on the left upper quadrant of abdomen and weight loss, was admitted to St. Paul's Hospital. Abdominal CT scan showed a 12 x 8 x 8 cm-sized cystic mass in the left upper quadrant of the abdomen. Endoscopic ultrasonography showed a large cystic mass between the stomach and the liver, which was 1.6 cm length in wall thickness. Laparotomy and resection of the mass was performed. A 12 x 8 x 8 cm-sized mass, originated from the lesser omentum, was discovered near the lesser curvature of the stomach. Microscopic examination revealed spindle-shaped cells with 7-8 mitoses per high power field. She was diagnosed as primary leiomyosarcoma originated from the lesser omentum.
